  1. At my own initiative, I have requested for the Introducer to refer me to AIA International Wealth Private Limited ("Company") as I wish to find out more about the financial products and services offered by the Company. The Company is a licensed financial advisor in Singapore and is authorised to provide the services as listed on https://eservices.mas.gov.sg/fid/institution/detail/424300-aia-international-wealth-private-limited.

  2. I understand that the Introducer's role is limited to making the referral. The Introducer is not permitted to market, make any recommendation, or provide advice on the financial products distributed by the Company. Instead, any queries relating to the financial products and services distributed by the Company may be directed to the Company's Wealth Management Consultant. The Introducer should also not handle any money in relation to the financial products.

  3. I consent to the collection, use, processing, and transfer of my personal data set out in this Customer Consent Form by the Company (including the transfer of the personal data to Singapore) for the purposes of the Company contacting me to provide me with the information on its financial products and services. If I purchase any insurance policy or other financial product distributed by the Company, I consent to the disclosure, use, processing and transfer of my personal data and information on the purchased product by the Company to the Introducer to facilitate the payment of fees to the Introducer.   1. I have requested for Citibank Singapore Limited ("CSL") to refer me to AIA International Wealth Private Limited ("Company") as I wish to find out more about the financial products and services offered by the Company. The Company is a licensed financial advisor in Singapore and is regulated by the Monetary Authority of Singapore to provide the services as listed on https://eservices.mas.gov.sg/fid/institution/detail/424300-aia-international-wealth-private-limited.

  2. I understand that CSL's role is limited to making the referral and introducing me to the Company. CSL is not permitted to market, make any recommendation, or provide advice on the financial products distributed by the Company, nor to arrange or negotiate insurance contracts on my behalf or on behalf of the Company. Instead, any queries relating to the financial products and services distributed by the Company may be directed to the Company's Wealth Management Consultant. CSL shall also not handle any of my money in relation to such financial products (save where I instruct payment from my CSL bank account directly to the insurer's designated bank account). I acknowledge that CSL is not an agent of the Company or vice versa and under no circumstances will either party be liable for the acts or omissions of the other. I have not relied on CSL for tax, accounting or legal advice relating to any of the products and services.

  3. I understand that CSL may receive a referral fee from the Company if I purchase any insurance product distributed by the Company and I hereby expressly and irrevocably consent to the payment by the Company and receipt by CSL of such referral fee. Any payment from the Company to CSL will not impact my cost of insurance. Further information about the referral fee may be disclosed by CSL upon my request.

  4. I understand that this referral arrangement is available to permanent full-time staff based in Singapore, who are employed by CSL and other Citigroup entities operating in Singapore.

  5. I consent to the Company collecting, using, processing, and transferring my personal data (including cross-border transfers) set out in this Customer Consent Form and in the HR letter certifying my employment with CSL and/or its affiliates that I provide in connection with this referral arrangement for the purposes of the Company (i) contacting me to provide me with information on its financial products and services; and (ii) administering this referral arrangement. I further consent to the Company disclosing, using, processing and transferring my personal data to CSL (including cross-border transfers) for the purposes of administering this referral arrangement, including (i) tracking the status and outcome of my engagement with the Company and the status of any application submitted by the Company on my behalf; and (ii) facilitating the payment of fees and related reconciliation reporting to CSL. Where I purchase any insurance policy distributed by the Company, such disclosure may include information relating to the purchased product.   6. I consent to CSL collecting, using, processing and transferring my personal data set out in this Customer Consent Form and received from the Company for the purposes of CSL: (i) tracking the status and outcome of the referral and my engagement with the Company including the status of any insurance policy application submitted by me in connection with the referral arrangement, (ii) facilitating the reconciliation of fees received from the Company in connection with this referral arrangement and (iii) internal management reporting, performance analysis, and administration of this referral arrangement. I further consent to CSL disclosing, using, processing and transferring my personal data to AIA Singapore Private Limited ("AIA") for the purposes of reporting on the referral arrangement to AIA including the number and status of referrals. I have also read, understood and agree that CSL may collect, use and disclose information about me in the manner described in the Privacy Circular (https://www.citibank.com.sg/global_docs/pdf/FINAL_CSL_PDPA_Circular.pdf, also available at Citibank website (Footer)>Privacy>Personal Data Protection and You>Privacy Circular). Further, if I am a foreign national/resident and where a data privacy circular applicable to my country of nationality/residency has been prepared by CSL (whether now or in the future) to address applicable data privacy requirements, I agree to the terms of such data privacy circular as set out in the Citibank Singapore Website (Website Footer > Privacy) which may be updated by CSL from time to time.
